Treatment Record Study Packet- This packet contains a sample Treatment Record and a list of orthodontic abbreviations. These forms should be used when studying the Treatment Record Course.

Treatment Record- This sample Treatment Record is the main form of communication in an orthodontic practice about a patient's treatment. This form includes Initial and Progress findings, Estimated Treatment Completion Date, and a detailed Treatment Plan that serves as a guide for the delivery of treatment for all members of the team. It may be computerized.

Abbreviations- This is a list of common abbreviations used in an orthodontic practice. Everyone on the team who enters notes on the Treatment Record should know the abbreviations used in the practice.

Abbreviations- This list of orthodontic abbreviations is in MS Word format so it may be adapted to the needs of the practice.
